Wales needs a structured, accountable system for endometriosis care that ensures every patient receives consistent, specialist‑led management. Health Boards must be required to follow NICE guideline 73 in full, with formal auditing, reporting duties, and consequences when guidance is not implemented. A national Endometriosis Registry is essential to capture diagnostic pathways, staging accuracy, symptom burden, surgical techniques, complications, recurrence intervals, fertility impact, and disability progression. This data would allow Wales to track outcomes, identify regional variation, and plan services based on real evidence rather than assumptions. Mandatory annual specialist reviews would ensure ongoing assessment of pain, organ involvement, treatment effectiveness, and risk of recurrence, preventing women from being abandoned without follow‑up. Current care lacks structure, oversight, and measurable outcomes. Without these reforms, Wales cannot deliver safe, equitable care or understand the true scale of harm caused by delayed diagnosis and inadequate monitoring.
