Petition Commission an independent review of patient safety during acute mental-health crises in Wales
I am calling for this review following two reported fatalities and a serious reported near miss involving people experiencing acute mental-health crises, including concerns at The Grange University Hospital. This petition is about patient safety and prevention: ensuring robust risk assessment, appropriate supervision, specialist mental-health input and safe access to care while respecting patients' dignity and legal rights.

One of the reported fatalities involved my best friend. I have separately provided evidence to the Coroner and raised patient-safety concerns with Healthcare Inspectorate Wales.
I am asking for an independent review of whether current systems are sufficiently robust, including risk assessment, consideration of capacity where relevant, information-sharing, specialist mental-health assessment, supervision and procedures when a vulnerable patient leaves an emergency department.
I believe these reported incidents highlight the need for an independent review of whether there are common risks or gaps in current systems for protecting people experiencing acute mental-health crises in Wales, so that appropriate lessons can be identified and further harm prevented.
